Ilial wing fracture

Pelvic stabilisation in a young domestic shorthair after polytrauma.

This sweet cat was brought in by a member of the public after being seen dragging her back legs. A charity centre kindly took on her case and supported her recovery, allowing us to help with her fractured hip.

Presentation & imaging

On presentation, Charlie was unable to bear weight on her pelvic limbs. Withdrawal reflex and nociception were present, with normal proprioception in the right pelvic limb but delayed in the left.

Radiographs confirmed:

  • Right sacroiliac luxation
  • Left oblique ilium body fracture
  • Left ischium fracture involving the acetabulum
  • Bilateral pubic fractures

Surgical plan & repair

To address her injuries, a 2.4 mm screw was placed to stabilise the sacroiliac joint, and a 2 mm SOP plate with two proximal and two distal screws was used to reduce the ilial fracture.

Outcome

Charlie made an excellent recovery and was using all four limbs during follow-up checks. An FHNE may be necessary in the future if the acetabular fracture leads to discomfort.
